Working families can apply for the new childcare support via an online form on the gov.co.uk website.

You will need to provide the following information to complete the form:

• Full name 
• Address
• National insurance number
• Whether you expect to meet the income requirements over the next 3 months
• Whether you are receipt of any benefits.

If you are eligible you will be given an 11 digit code that you will need to provide to your childcare provider when applying for your child's space. You will also need to provide your national insurance number and your child's birth certificate or passport as proof of age.
